2007 Course Registration

Advanced Body Sculpting Institute requires a completed registration form, as well as copies of the following necessary documents from each course applicant.

• Medical License
• Passport and/or Drivers License
• CV (curriculum vitae)
• Health Status Report
• Professional Liability Carrier
• Surgical Residency Certificate

Note: Other documents may also be required depending on course location and/or facility.

Your completed registration form, necessary documentation, and course fees will ensure that your registration is processed as quickly as possible.

Incomplete submissions will not be processed until all required documents and payment are submitted and approved. Any delays in submissions may delay course reservations.
